The Continuity of Care Gap: Effective Communication and Transition

As providers know, SUD is a chronic, not acute disease that can be effectively treated by a distinct episode of care. Our patients require a continuity of care as they proceed through the treatment and recovery continuum. Yet it is difficult, and we sometimes fail to communicate and execute transitions from stages of care. Nor have we yet effectively established adequate recovery communities. Hear from members of the care transition providers about the process and how we can do better.

Marcie Chambers received her PhD from the University of Colorado Boulder. As a college professor and with a private practice, she saw many clients going in and out of addiction treatment centers and psychiatric wards. In 2007, she developed an extended care, dual diagnosis residential treatment program for women named The Rose House. The six-bed facility has grown into a twenty-eight-bed program offering a continuum of care. Using an evidence based integrative approach, The Rose House treats adult women for addiction, anxiety, depression, mood disorders, and trauma. Dr. Chambers serves as the CEO of The Rose House.

Jim Geckler is a native of Buffalo, NY who has served the recovery community in a professional capacity for almost 20 years. His experience in addiction treatment has touched all phases of the continuum of care, from establishing the first LGBTQ+ recovery residence in Minneapolis to leading families through the process of intervention as Chief Operating Officer of Addiction Intervention Resources, to multiple leadership roles at the Hazelden Betty Ford Foundation. As CEO at Harmony Foundation, Jim has focused on increasing their capacity to meet the unique needs of people who struggle with substance use disorder.

Phil Rutherford is the Director of Operations of Faces and Voices of Recovery and possesses a self-described Doctorate from the school of Hard Knocks. Phil has a BA in Psychology with a specialization in SUD. Phil’s recent experience as Director of Operations at Recovery Is Happening in Rochester, MN offered Phil a front-row seat into the world of Recovery Community Organizations. Prior to that, Phil spent his career in management at Microsoft, Micron Electronics, and Taylor Corporation. Phil is an active member of the recovery community and has considerable experience in Substance Use Disorders, Recovery, and Re-entry.

Nanette Zumwalt is President and CEO of Hired Power and recognized as one of the top interventionists and an industry expert. Nanette partnered with Dr. Judith Landau creating the International Recovery Institute. Together they developed a curriculum for an imperative skills training awards including from California Assemblyman Jim Morrissey. Nanette as an International Certified Alcohol and Drug Counselor Level II Clinical Supervisor, an International Certified Criminal Justice Professional, a Certified Intervention Professional, Certified ARISE Interventionist, and a member of the Association of Intervention Specialists. Nanette currently serves on the Board of Directors for NAATP, and past board member of Association of Intervention Specialists.
